What Is End-of-Life Care at Home

End-of-life care at home supports people who are living in their final stages of life with a life-limiting or terminal illness, helping them remain comfortable and cared for in their own homes. It can begin weeks or months before death, and individual care plans adapt over time as needs change, with care typically focussed around: 

  • Managing pain & physical symptoms 
  • Medication management 
  • Coordinating with healthcare professionals 
  • Supporting emotional & mental wellbeing 
  • Ensuring personal wishes, routines & beliefs are respected 
  • Supporting families through anticipatory grief and loss 
  • Helping with daily tasks like meal prep, housekeeping, and personal hygiene 

An increasing number of families are choosing for this final chapter of life to be spent together at home, with around 1 in 4 people choosing to die at home in recent yearsThis number is continuing to rise, as for many people, home is where they feel most themselves, surrounded by familiar comforts and the people they love most.

Cancer end of life care 

Advanced or aggressive cancers can bring a whole number of different complex symptoms such as pain, fatigue, nausea, breathlessness, and anxiety. We commonly support people in the final stages of lung cancer, bowel cancer, and breast cancer, alongside many other diagnoses. Our carers support with pain management routines, comfort positioning, medication support, personal care, and emotional reassurance, helping people feel as settled and comfortable as possible at home. 

Dementia end of life care 

As dementia reaches its later stages, your loved one may experience increased agitation, difficulty swallowing, reduced mobility, and challenges communicating their needs. Our carers provide calm, respectful support, focusing on comfort, personal care, and maintaining routines that reduce distress for both the individual and their family. 

Motor neurone disease (MND) end of life care 

Motor neurone disease often leads to progressive muscle weakness. This can also cause difficulties for your loved one with speech, swallowing, and breathing, which can feel very frightening to deal with if you are the main person looking after them. Our carers are trained to support complex physical needs, assist with communication, manage comfort and positioning, and work alongside healthcare professionals to ensure care remains responsive and compassionate, throughout the day and night. 

COPD end of life care 

Living with the final stages of advanced of COPD can mean persistent breathlessness, fatigue, anxiety, and frequent flare-ups that can feel very scary for both your loved one and you. End-of-life care at home focuses on easing breathing discomfort, supporting medication routines, and creating a calm environment that reduces panic and exhaustion. 

Stroke end of life care 

Following a severe stroke, your loved one may experience limited mobility, swallowing difficulties, pain, and changes in awareness and communication. Our carers provide sensitive, hands-on support with personal care, comfort, repositioning and nutrition to help families manage complex needs at home. 

Parkinson’s disease end of life care 

In the later stages of Parkinson’s, symptoms such as rigidity, pain, swallowing difficulties, hallucinations, and reduced independence can become more pronounced. Our carers confidently support with medication routines, mobility assistance, comfort measures, and emotional support, helping reduce distress and maintain dignity. 

Old age 

Being in the familiarity of home, surrounded by loved ones can bring some peace in your loved ones final phase of life. Our dedicated carers strive to make them as safe and comfortable as possible to facilitate this for the family, so they can spend time with their loved one, rather than worrying about day-to-day care

It can feel incredibly overwhelming for a family trying to provide home care for someone in their final months, weeks or days. Whilst loved ones may wish to remain at home, surrounded by familiar comforts and the people they love, knowing how to make that possible as their carer can feel incredibly daunting.

Visiting Care for End of Life 

Visiting carers come at agreed times throughout the day to provide essential support such as giving medicines, personal care, assistance with mobility, comfort measures, and emotional reassurance. This means your loved one can remain in familiar surroundings while giving family members guidance, reassurance, and time to focus on spending more quality time together. 

Overnight Care for End of Life 

Carers can come and stay in your home overnight to provide a reassuring presence during the quiet hours when your loved one or family members can feel most isolated or anxious. Our carers ensure your loved one is comfortable, manage pain or symptoms, assist with toileting or repositioning, and respond promptly to any needs or emergencies that may arise. This allows you to sleep with confidence, knowing your loved one is safe and has company. 

Live-In Care for End of Life 

For people with more intensive needs, a live-in carer can provide continuous support, blending seamlessly into daily routines while offering constant stability and quality care. This ensures your loved one’s needs are met around the clock, providing peace of mind for everyone.
